Initial Consultation consists of a one-on-one virtual appointment with Dr. Chambers to discuss your desires and outcomes. If you decide to move forward with evaluation and treatment planning, you will be provided consents and other documentation to return prior to your first treatment session. Initial application will be provided upon booking your consultation session. Please make sure you use an email that you can access. Information must be returned within three days of your consultation. Payment must be made at the time of booking.
$150 - One Hr Initial Consultation
Evaluation and treatment planning takes approximately two hours, one-on-one virtually, to secure a comprehensive health background. Treatment planning will take place following the information gathering evaluation and Dr. Chambers will provide you with a comprehensive integrative treatment plan within two weeks of all labs, collateral contact, and information gathered and/or received. A one-hour report of recommendations is included in the cost, post completion of your individualized plan. Collateral contact, referral for labs, referral to additional medical providers, and receipt and review of pertinent health information is included in the total cost. However, additional costs for labs, other providers to obtain your records, etc…may be required by those providers.
$850 - two hr evaluation, treatment plan, one hour recommendation session
A 3-6 month follow-up appointment is available if you want to fine tune, update, or further discuss your treatment plan. If alternative follow-up is needed, options are available. Ongoing follow-up services to maintain and update your treatment needs can be provided, as well, on an individualized basis for need.
